Update, October 28, 2026
Lilly posted topline results from TRANSCEND-T2D-2 on October 28, 2026. The trial is NCT06260722, an open-label comparison of once-weekly retatrutide 9 mg and 12 mg with Ozempic 2 mg in 1,298 adults with type 2 diabetes on metformin, with or without an SGLT2 inhibitor. On the efficacy estimand, which assumes people stayed on the assigned drug, A1C fell 2.2% with both retatrutide doses versus 1.8% with Ozempic at 80 weeks. Body weight fell 19.5% at 9 mg and 20.9% at 12 mg, versus 9.4% with Ozempic. Discontinuation for adverse events was 4.8%, 7.9% and 2.7% in that same order. Lilly said the full paper is not out yet, that retatrutide is still investigational, and that nobody is allowed to sell it. The release does not announce a US filing. The Q1 2027 plan from July 23, 2026 is still the filing date on the record (Lilly, October 28, 2026).
When will retatrutide be available?
Not in 2026. I would not plan on a US pharmacy filling a prescription for it this year.
Lilly's public timing is still a US submission in the first quarter of 2027, after the manufacturing and quality package. The July 23, 2026 release called that filing a biologics license application, or BLA (Lilly).
FDA's published review goals, once an application is filed and accepted, are 10 months for standard review or 6 months for priority review, after the 60 days FDA has to accept the file (FDA PDUFA goals). A first-quarter 2027 submission would likely mean a decision in late 2027 with priority review or early 2028 with standard review. That is our estimate from those goals, not a Lilly date. I have not seen an announcement of priority review or a Commissioner's National Priority Voucher for retatrutide.
A pharmacy launch comes after a decision, and only after supply is ready. Foundayo, Lilly's orforglipron pill, was approved on April 1, 2026, and CNBC reported a US launch that same month (CNBC, October 16, 2026). A fast launch on a different drug is not a schedule for this one. How Foundayo compares with the Wegovy pill is in Foundayo vs the Wegovy pill.
The filing type is the open argument. Lilly asked FDA to review retatrutide as a biological product under a BLA. On October 2, 2026, FDA wrote that retatrutide is a drug, not a biological product, so an application would go under section 505 of the Federal Food, Drug, and Cosmetic Act. That letter is Document 46-2 in Eli Lilly and Company v. Robert F. Kennedy, Jr., No. 26-1301 (7th Cir.), filed with the court on October 8, 2026 (FDA letter; docket). The appeal was argued on September 24, 2026. The docket I checked does not show a decision. Nothing in the letter, or in Lilly's July release, moves the filing quarter.
No Canadian filing date has been announced. Once a filing exists, Health Canada's review targets are in US, Canada and other countries.
Pete's take: The highest dose in TRIUMPH-1 averaged 25.0% weight loss on the treatment-regimen estimand, which counts people who stopped the drug. Finished phase 3 is a trial result. It is not a product a pharmacy can dispense. Anything sold as retatrutide outside a trial is illegal, and the death reports in Latest are why I will not dress that up.
Availability
Retatrutide is not FDA approved. As of October 28, 2026, it is available only in clinical trials. There is no approved product a pharmacy can dispense. Registry rows for the other late-stage drugs checked for this site are on coming soon.
United States. FDA has not approved retatrutide. Eli Lilly plans to submit a US application in the first quarter of 2027. A physician-requested expanded-access study exists for a narrow group of adults (NCT07629401). That study is not a product on the market. Country-by-country status is in US, Canada and other countries.
United Kingdom. On October 15, 2026, the Medicines and Healthcare products Regulatory Agency (MHRA) said retatrutide has not been authorised for use in the UK. Products being sold as retatrutide outside legitimate clinical trials are being supplied illegally (MHRA).
Latest, October 15, 2026
Retatrutide is still investigational. Lilly's stated plan is still a US submission in the first quarter of 2027. The October reports below did not announce that the application has been filed.
Two death reports about products claimed to be retatrutide
CBS News reported on October 13, 2026 that FDA had received two reports, in recent months, of people who died after taking bootleg products claiming to be retatrutide. Becker's Hospital Review carried that same account the same day. Both reports were submitted to FDA's Adverse Event Reporting System (FAERS). The documents and Eli Lilly say neither report is connected to a formal clinical trial.
In July, a healthcare professional told FDA that a 41-year-old woman with obesity went into cardiac arrest. The report lists retatrutide as a suspect product obtained from an unreliable source. She reached a hospital with a dangerously fast heartbeat and died after resuscitation failed. The report also said she was drinking kava.
In September, FDA received a report that a 49-year-old patient died after severe dehydration, malnutrition, and cardiac arrest. The report says a licensed nurse convinced her to buy a product sold as retatrutide and sold her vials from an online peptide vendor.
FDA declined to say whether it has verified the information in either report. CBS News said it could not independently verify the reports, which it obtained under the Freedom of Information Act. A FAERS report does not prove the product caused or contributed to the death.
Eli Lilly said the reports "make clear the two individuals were taking black-market retatrutide." Lilly said it has found suspect products "with too much, too little, or the wrong active ingredient," along with contaminants that "can put patients at serious risk."
Warning letters since December 2024
JAMA Medical News reported on October 9, 2026 that FDA has issued 21 warning letters, dating back to December 2024, to entities selling retatrutide (Rubin, JAMA, doi:10.1001/jama.2026.20514). That figure is JAMA's report. This update does not rebuild the count from FDA.gov.
Symptoms described on social media
JAMA Network Open published a study on October 14, 2026 of Reddit posts from people who said they were using retatrutide outside an approved product (Sehgal et al., doi:10.1001/jamanetworkopen.2026.38929). Among 7,823 users with at least one mapped symptom term, gastrointestinal symptoms were common but not dominant, which differs from the phase 2 trial, where nausea, diarrhea, vomiting, and constipation were the most common adverse events. The posts most often described increased appetite (1,336 users, 17.1%), fatigue (1,188, 15.2%), increased energy (970, 12.4%), insomnia (475, 6.1%), a higher heart rate (374, 4.8%), and muscle and connective-tissue terms (1,125, 14.4%). Nausea was still common (907, 11.6%). A higher heart rate in unverified posts is not the Wegovy label. What that label says about resting heart rate, and how three rings track it, is in Oura vs WHOOP vs Ultrahuman on a GLP-1.
The authors call the findings hypothesis-generating. The posts cannot show how often these symptoms occur, whether a product caused them, or how they compare with approved GLP-1 drugs. Product identity, purity, and dose could not be verified, and people who post about problems may differ from everyone using these products.
Lawsuits, with the filing plan unchanged
On October 8, 2026, Eli Lilly filed complaints against five more companies, alleging they sold and promoted illicit copies of retatrutide (Bloomberg Law; Washington Post, October 9, 2026). The Washington Post said these cases follow six lawsuits Lilly filed in August. The complaints are allegations.
The filing plan on record is the one Lilly stated on July 23, 2026: a US submission in the first quarter of 2027, after the manufacturing package. None of the October reports checked for this update announced a filing or a new target quarter.
"I thought this was still on clinical trials." That comment, posted under a forum thread about buying retatrutide, is closer to the truth than most of the thread. Retatrutide is not approved anywhere in the world. The phase 3 results are in, and some are already in major journals. The application that starts FDA's review clock has not been filed.
This retatrutide FDA approval tracker lists every dated step so far, the possible decision windows, and the readouts still to come.

When could FDA approve retatrutide?
FDA review goals are set by the PDUFA agreement. After an application arrives, FDA has 60 days to accept it for review. Then it aims to decide within 10 months (standard review) or 6 months (priority review) (FDA PDUFA goals).
| Pathway | Review goal after filing | If Lilly submits in Q1 2027, a decision would land around |
|---|---|---|
| Standard review | Review goal after filing60 days + 10 months | If Lilly submits in Q1 2027, a decision would land aroundQ1 2028 |
| Priority review | Review goal after filing60 days + 6 months | If Lilly submits in Q1 2027, a decision would land aroundQ3 to Q4 2027 |
| Commissioner's National Priority Voucher | Review goal after filing1 to 2 months (pilot program) | If Lilly submits in Q1 2027, a decision would land aroundPossibly mid-2027; no voucher announced for retatrutide |
The voucher row is not hypothetical for this drug class. FDA approved Lilly's orforglipron pill (Foundayo) on April 1, 2026, 50 days after filing, under that program (FDA). How that pill compares with the Wegovy tablet is in Foundayo vs the Wegovy pill. We found no announcement of a voucher or priority review for retatrutide. The PDUFA agreement covers fiscal years 2023 to 2027, and a new agreement could change these goals.

What the phase 3 trials found
Retatrutide is a once-weekly injection that activates three hormone receptors: GIP, GLP-1 and glucagon. Semaglutide activates one of those, and tirzepatide two.

Side effects. In TRIUMPH-2, at the highest dose versus placebo:
- diarrhea: 33.6% vs 13.2%
- nausea: 28.0% vs 8.0%
- dysesthesia, an abnormal skin sensation such as tingling or burning: 7.3% vs 0.7%
Those rates come from a diabetes trial. People already on an approved GLP-1 who are dealing with nausea or diarrhea often compare electrolyte drinks by sodium and sugar.
Heart outcomes. TRIUMPH-3 was not designed to prove heart benefit. Its major-adverse-cardiac-event results had wide confidence intervals that crossed 1 (MACE-5 hazard ratio 0.82, 95% CI 0.55 to 1.22), so they show neither benefit nor harm. This was not a home-monitoring study. Readers checking blood pressure on an approved GLP-1 can compare upper-arm home blood pressure monitors.

US, Canada and other countries
United States. Investigational and not approved. FDA says retatrutide cannot be used in compounding under federal law. It has also sent warning letters to sellers marketing it, including products labelled "for research purposes" or "not for human consumption" (FDA). JAMA Medical News reported on October 9, 2026 that FDA has issued 21 warning letters to retatrutide sellers since December 2024. The October 15 update is in Latest.
Expanded access. Lilly's program (NCT07629401) is for individual adults with a body mass index of 35 or higher and at least two serious obesity-related complications, who have not responded to approved treatment at its highest dose and cannot join a trial. A treating physician makes the request.
Canada. Not authorized by Health Canada, and we found no announced Canadian filing date. Health Canada's review target for a new drug submission is 300 days, or 180 days under priority review (Health Canada performance standards).
Other countries. Lilly Canada stated in August 2026 that no regulator anywhere has approved retatrutide. Check your national regulator for updates.
What is in an unapproved vial. Nobody can verify it. When researchers test-bought semaglutide from online sellers without a prescription, measured purity ran from 7.7% to 14.37% against a labelled 99%, and every vial they tested contained bacterial endotoxin (Ashraf et al., 2024, PMID 39509151). That study did not test retatrutide.
On an approved GLP-1 medicine now? Two worries come up again and again with these drugs: eating enough protein when appetite is low ("How do you eat enough protein when you don't want to eat anything?"), and keeping muscle. Tools readers use to track both:
- An unflavored protein powder that mixes into food. Protein powders compared by cost per 30 g.
- A body-composition scale to watch trends, not single days. Smart scales vs DEXA. (confirm program approval)
- A food-tracking app with a protein target (confirm program approval)

Retatrutide FDA approval status in October 2026
The search "retatrutide FDA approval status 2026" is looking for a yes, or for a calendar date. There is neither.
As of October 28, 2026, FDA has not approved retatrutide. Eli Lilly has not submitted the US application. The October 28 topline is in Update. The plan on the record is still a filing in the first quarter of 2027, after the manufacturing and quality package (Lilly, July 23, 2026).
On October 2, 2026, FDA wrote that retatrutide is a drug, so an application would go under section 505 of the FD&C Act, not a biologics license application. That letter is in the Seventh Circuit record. It does not say the file has been submitted, and it does not move the quarter (FDA letter).
Pete's take: Finished phase 3 is a result. It does not start FDA's review clock. The clock starts when FDA accepts an application. That has not happened.
What retatrutide availability means
"Retatrutide availability" gets used for three different things. Only the first one is a real route in October 2026.
A clinical trial. That is the legitimate supply. A trial slot is not a prescription a pharmacy refills later.
Expanded access. Lilly registered NCT07629401 in June 2026. It is for adults with a BMI of 35 or higher, at least two serious obesity-related complications, no response to approved treatment, and no trial they can join. A treating physician makes the request. It is not a product launch.
A pharmacy. Not in 2026. Lilly Canada said in August 2026 that no regulator anywhere has approved retatrutide. On October 15, 2026, the UK's MHRA said products sold as retatrutide outside legitimate clinical trials are being supplied illegally (MHRA).
FDA also says retatrutide cannot be compounded. JAMA Medical News reported 21 warning letters to sellers since December 2024. I would not treat a group-chat vial as availability. The two FAERS reports about products claimed to be retatrutide are in Latest. Those reports do not prove a product caused the deaths. They are a reason to treat anything outside a trial as unknown.
Common questions
Is retatrutide FDA approved?
No. As of October 28, 2026, retatrutide is investigational and not approved by FDA or any other regulator. Eli Lilly plans to submit its US application in the first quarter of 2027.
When will retatrutide be available?
Not in 2026. Lilly plans to file in the US in the first quarter of 2027. On FDA's published review goals, a decision would likely come in late 2027 with priority review or early 2028 with standard review, and a launch would follow that decision. This is our estimate, not a Lilly date.
When will Eli Lilly release retatrutide?
Lilly has not given a launch date. Its only public timing is a US submission in the first quarter of 2027, after the manufacturing and quality package.
Can I get retatrutide now?
Only through a clinical trial or the expanded-access study NCT07629401, listed above. FDA says retatrutide cannot be compounded. On October 15, 2026, the UK's MHRA said products sold as retatrutide outside legitimate clinical trials are being supplied illegally.
Has Lilly filed for retatrutide approval?
Not as of October 28, 2026. Lilly said on July 23, 2026 that it would submit in Q1 2027, after completing the manufacturing and quality package the application requires. The October 28, 2026 TRANSCEND-T2D-2 release did not announce a filing. FDA's October 2, 2026 letter describes the pathway, section 505 rather than a BLA, and does not announce a filing or a new quarter.
Is retatrutide approved in Canada?
No. Health Canada has not authorized retatrutide, and its April 2026 advisory lists it among unauthorized injectable peptides it has seized.
Can retatrutide be compounded?
No. FDA says retatrutide cannot be used in compounding under federal law. JAMA Medical News reported on October 9, 2026 that FDA has issued 21 warning letters to retatrutide sellers since December 2024.
When will retatrutide vs tirzepatide results come out?
ClinicalTrials.gov lists TRIUMPH-5's primary completion as November 2026 (estimated). Lilly usually announces topline results within a few months of completion, so early 2027 is plausible. That timing is our estimate.
Can I get retatrutide through expanded access?
Lilly registered an expanded-access program in June 2026 (NCT07629401) for adults with a BMI of 35 or more, at least two serious obesity-related complications, no response to approved treatment and no trial option. Requests go through a treating physician.
What is the retatrutide FDA approval status in 2026?
Not approved. As of October 28, 2026, Eli Lilly has not submitted a US application. The company plans to file in the first quarter of 2027. On FDA's published review goals, a decision would typically land in late 2027 with priority review or early 2028 with standard review. That timing is our estimate, not a Lilly date.
Is retatrutide available in 2026?
Not as a prescription. Availability in October 2026 means a clinical trial, or the narrow expanded-access study NCT07629401. A pharmacy cannot dispense an approved retatrutide product, because none exists.
Is retatrutide available outside a clinical trial?
Not on the record we checked. FDA says it cannot be compounded. The UK MHRA said on October 15, 2026 that products sold as retatrutide outside legitimate clinical trials are being supplied illegally. Lilly Canada said in August 2026 that no regulator has approved it.
Has the retatrutide FDA approval status changed this month?
No filing and no approval. The October 2026 updates were warning letters, lawsuits, two FAERS reports about products claimed to be retatrutide, an FDA letter on the application pathway, and the MHRA statement. None of them announced that Lilly has submitted the application.
